About this role
The Home Health Triage RN provides after-hours telephonic and in-person nursing care for members of the On Lok PACE Health Plan at Institute on Aging, supporting aging adults and adults with disabilities. The role operates within IOA’s care model to help participants maintain health, independence, and age in place while connecting with interdisciplinary teams and primary care providers.
