Home Visitor

This role involves recruiting and maintaining a caseload of children and pregnant mothers, conducting regular home visits, and developing individualized family support plans. The Home Visitor collaborates with parents and community partners to facilitate educational activities, complete developmental screenings, and ensure program enrollment and documentation requirements are met. The position requires strong communication skills, cultural sensitivity, and the ability to work independently in both office and home settings.
