Hospice Case Manager

Capstone Hospice is seeking a compassionate and experienced Registered Nurse to serve as a Hospice Case Manager, providing end-of-life care and clinical support to patients and families in the Smyrna/Marietta area. The role involves conducting patient assessments, developing individualized care plans, managing symptoms, and educating families on the hospice process. This full-time position offers autonomy in schedule management while maintaining a typical caseload of 14-16 patients.
