Hospice Clinical Liaison

The Hospice Clinical Liaison will manage intake and referral processes for hospice patients, ensuring eligibility, documentation, and admission requirements are met. The role involves building relationships with hospitals, physicians, case managers, and other referral sources, as well as educating patients and families about hospice services. The liaison will also conduct daily visits to facilities to maintain visibility and support seamless handoffs to the hospice care team.
