Hospice Liaison (English/Spanish)

The Hospice Liaison promotes Silverado’s hospice services by building and maintaining relationships with referral sources, medical professionals, and the community to connect patients and families with appropriate end-of-life care. This mission-driven role combines sales and community outreach responsibilities, supports census development through qualified referrals, and represents the organization in the San Diego area. The position requires travel within a designated territory and collaboration with internal teams to ensure high-quality patient access and support.
