Hospice Liaison RN

The role serves as a liaison between the organization and hospital staff, educating team members on services and programs while facilitating communication and collaboration with physicians and interdisciplinary teams in the inpatient setting. It leads strategic initiatives to increase referrals, coordinates referrals, and provides case management for hospitalized hospice patients, reporting directly to the Executive Director.
