Hospice LPN

This full-time Licensed Practical Nurse position provides end-of-life care and hospice services to patients in the community setting under routine or continuous care. The role involves assisting in planning patient and family care under the direction of the RN Case Manager, delivering skilled nursing care, and reinforcing appropriate care techniques. The position requires working independently to manage patient symptoms, administer medications, and participate in interdisciplinary team meetings.
