Hospice LPN

Croí Health is seeking an experienced and compassionate Hospice LPN to provide skilled nursing services to patients in their homes across the South Shore. The role involves collaborating with the RN case manager to deliver direct patient care and communicate any changes in status to the physician and case manager. This position requires strong clinical assessment skills, the ability to prioritize and triage patients, and excellent communication skills to work effectively within a team structure.
