About this role
The RN Case Manager provides hands-on hospice and palliative nursing care and coordinates individualized plans of care for patients and families in the Coastal Valley (Riverside, Banning, Beaumont, Moreno Valley) service area. This clinician collaborates with interdisciplinary teams and attending physicians to ensure quality end-of-life care and supports field staff such as LVNs and Home Health Aides. The role requires travel to patient locations and flexibility to work outside regular hours as needed.
