Hospice RN Case Manager

About this role
The RN Case Manager coordinates and delivers hospice and palliative nursing care for patients and families across the North Houston service area. The clinician develops individualized plans of care, works with interdisciplinary teams and attending physicians, and provides field-based case management and supervision of LVNs and aides. This role requires travel to patient homes and flexibility to work outside regular hours as needed. The position is part of Silverado’s clinical nursing team focused on compassionate end-of-life care.
