About this role

This role is for an On Call/After Hours Registered Nurse responsible for providing skilled nursing assessments, interventions, and supportive care for hospice and home health patients outside of normal business hours. The nurse must manage rotating evening, night, weekend, holiday, and on-call shifts to address clinical and non-clinical situations while ensuring compliance with federal and state regulations.

Responsibilities

Triage calls from patients and caregivers during after-hours shifts
Develop and implement comprehensive care plans for hospice and home health patients
Conduct timely home or facility visits to evaluate and update care plans
Educate patients and families on pain management and disease processes
Communicate critical patient changes to hospice teams and supervisors

Qualifications

Current RN License in OhioBSN in Nursing
