Hospice RN Per Diem

This part-time role coordinates and manages a caseload of hospice patients, providing comprehensive nursing care and assessments in conjunction with the interdisciplinary team. The position involves supervising Licensed Nursing Assistants, developing plans of care, and ensuring continuity of patient services through collaboration with internal and external resources. Per diem responsibilities include performing routine, initial, and on-call visits while adhering to Medicare Hospice Conditions of Participation.
