Hospice Social Worker

As a hospice social worker, you will provide emotional and logistical support to patients and their loved ones during an important and vulnerable time, ensuring their affairs are in order and honoring their wishes. You will help loved ones navigate the grieving process and make a meaningful difference in the lives of patients and families through actions ranging from granting end-of-life wishes to listening at the bedside. The role involves working primarily in patients' homes or care facilities, with weekly office meetings to collaborate with an interdisciplinary team.
