HOT Bayside Region Case Manager

The HOT Bayside Region Case Manager provides case management services, life skills support, and resource connections to individuals experiencing unsheltered homelessness, helping them move toward safe, stable housing and greater self-sufficiency. This role involves building trusting relationships, setting goals, and connecting clients to essential resources such as shelter, healthcare, and employment opportunities. The position requires a trauma-informed and strengths-based approach to empower clients and overcome challenges throughout their journey.
