House Calls Nurse Practitioner

The House Calls Advanced Practice Provider is responsible for the complex clinical management of patients in a home setting, providing total care in collaboration with the Medical Director and leadership team. The role ensures timely, patient-centered, and evidence-based medical care is delivered while advocating for patient needs. The position involves preventing unnecessary emergency department utilization through in-home services and addressing quality measures and health maintenance concerns. The APP will also make referrals to community resources and participate in interdisciplinary meetings to improve patient outcomes.
