Responsibilities

Act as a trusted key worker coordinating wraparound support plans for complex clients
Build sustained relationships to help service users stabilize and access accommodation
Liaise with multi-agency partners to secure support and deliver dynamic intervention plans
Manage crisis situations while maintaining safety and acting within data sharing frameworks
Develop effective working relationships with staff in community, voluntary, and statutory services
