Housing Support Case Manager

The role provides trauma‑informed intensive case management to youth, women, and families experiencing chronic homelessness, helping them secure permanent housing and offering ongoing support for up to 24 months. The incumbent works independently and as part of an interdisciplinary team, delivering services in offices, shelters, hotels, and community settings.
