Human Services Analyst

This position provides medical case management to individuals infected and affected by HIV/AIDS within a four-county area, conducting comprehensive needs assessments and developing individualized care plans. The role involves monitoring client adherence to treatment, coordinating linkages to community and government services, and maintaining accurate documentation in designated databases. Case managers also provide counseling, education, and advocacy to support clients in achieving their health and wellness goals.
