IMH Social Service Worker

The IMH Social Service Worker intervenes with patients in the Inpatient Rehab department who have complex social needs, assisting with eligibility determination for social programs and community assistance. The role provides crisis intervention for patients and families with psycho-social needs and coordinates the development of discharge plans for high-risk populations. This position collaborates with interdisciplinary teams to address emotional, social, and financial consequences of illness while ensuring timely and appropriate post-discharge care linkages.
