Implementation Manager

The Implementation Manager for Evergreen Connected Care owns the day-to-day execution of practice onboarding and manages ongoing operating relationships with partner practices. This role ensures timely launches by running the standard onboarding sequence, facilitating meetings, and coordinating parallel implementation and clinical workstreams. The position serves as a primary point of contact for partner practice leadership and administration while driving processes to closure and maintaining clear communication across all stakeholders.
