Southwestern Ontario Aboriginal Health Access Centre
The Indigenous Patient Navigator provides culturally safe support to Indigenous individuals and families as they navigate hospital, healthcare, and community services. Working within a Western health environment, the Navigator advocates for patients, coordinates care transitions, connects clients with Traditional Healers and community resources, and helps ensure equitable, respectful, and holistic care outcomes.
We provide high quality, wholistic health and wellness services throughout Southwestern Ontario. Services are provided to Indigenous (First Nation, Métis, Inuit) people and their families, living on and off-reserve, in rural areas, and in the urban Indigenous communities in London, Owen Sound, Newbury, Waterloo Wellington and Windsor.
