Intake Case Manager

Developmental Pathways is seeking an Intake Case Manager to join the Systems Navigation Team, delivering compassionate, person-centered service and support coordination to help individuals access Medicaid Waiver and State General Fund services. The role involves conducting in-home assessments, engaging with families and community providers, and collaborating with team members to enhance service access. This hybrid position requires flexibility to work remotely, in-office, and in the field while traveling within the catchment area to support essential responsibilities.
