The Transitions of Care (TOC) Integration Specialist ensures continuity of care for patients transitioning between facilities and home by providing advocacy, education, and case management. This role focuses on medication reconciliation, risk assessment, and using evidence-based tools to prevent readmissions and improve health outcomes for diverse populations. The specialist collaborates with hospital staff, discharge planners, and care facilities to resolve care gaps and supports patients and families with navigating health systems and resources.

Responsibilities

Conduct outreach to patients within two business days post discharge
Perform risk assessments and root cause analysis for readmissions
Facilitate medication reconciliation with nursing staff
Document all patient activities and complete monthly outcome reports
Collaborate with interdisciplinary teams to coordinate care transitions
