Intensive Care Coordinator

The Intensive Care Coordinator is responsible for coordinating individualized, community-based services for children and adolescents with Serious Emotional Disturbances using a wraparound, family-centered approach. This role involves facilitating team-based planning, service coordination, and cross-system collaboration to support youth and families in achieving stability and meaningful life goals. The position requires working with youth at varying intensity levels to ensure well-integrated care tailored to each individual's strengths and needs.
