Intensive Case Manager Baby CAT

The Intensive Case Manager Baby CAT provides intensive case management and wraparound services to families with children under five who are at risk of entering or currently in the child welfare system. This role focuses on stabilizing children and parents by addressing mental health and substance abuse concerns through trauma-informed, culturally competent interventions. The position requires collaboration with an Integrated Decision Team to develop individualized treatment plans and ensure family safety in home, school, and community settings.
