Intensive Case Manager Program: Parole Support and Treatment Program

The Intensive Case Manager will provide assessments, develop and monitor individualized service plans, and deliver supportive housing and clinical services to individuals under parole supervision with serious mental illness and co-occurring substance abuse disorders. The role involves regular client visits, coordination with housing counselors, and documentation in CAIRS and AWARDS systems to facilitate stabilization and transition to long‑term community housing.
