Intensive Case Manager

The Intensive Case Manager provides trauma‑informed case management to youth, women, and families experiencing chronic homelessness, helping them secure permanent housing and offering support for up to 24 months. The role involves community outreach, partnership with agencies, and hands‑on assistance with appointments, viewings, and home visits, while working independently and as part of an interdisciplinary team.
