Intensive Community Care Manager

The Intensive Community Care Manager (ICCM) is a Registered Nurse who provides hyperfocus case management and field nursing interventions to prevent unnecessary hospital arrivals for high-complexity patients. This role serves as a clinical lead for the Complex Care Team, assessing and coordinating care plans to stabilize patients and maximize their time at home. The position involves supervising team members, collaborating with primary care physicians, and ensuring regulatory compliance and quality patient care.
