Intensive Services QMHP- CS Access and Diversion

This role serves as an Access and Diversion Qualified Mental Health Professional within the behavioral health division, providing crisis assessments, interventions, and follow-up services to individuals at the LifePath Systems Diversion Center. The position focuses on connecting community members to appropriate care, diverting them from emergency rooms and jails, and addressing social determinants of health through collaboration with internal teams and external partners. The role requires a trauma-informed, person-centered approach to ensure timely access to outpatient behavioral health services and reduce inpatient or law enforcement interventions.
