Ithaca Shelter Case Manager

The Case Manager works directly with residents of the single adult emergency shelter to support them in developing individualized goals and successfully transitioning from homelessness to permanent housing. This role emphasizes person-centered, strengths-based, and trauma-informed care practices to ensure services are delivered with dignity and respect. The position involves providing crisis intervention, creating transition plans, and connecting residents with essential medical, mental health, and employment services.
