LCSW Full-time

The Social Worker provides emotional and psychosocial support, education, therapy, counseling, care coordination, discharge planning, and advocacy to patients, families, and vulnerable populations. Working within a multidisciplinary team, they conduct assessments, develop treatment plans, and utilize evidence‑based interventions across individual, group, and crisis settings while coordinating with insurers and community resources.
