Lead Care Manager, ECM

The Lead Care Manager provides direct service and coordinates care management as part of a Care Team for the Enhanced Care Management Program. This role involves overseeing cases, coordinating health care benefits, and facilitating client access to care while collaborating with clients, families, and providers to promote wellness and empowerment. The position also serves as an advocate and liaison with external health and social service providers.
