Lead Intake Admission Clinician

The Lead Intake Admission Clinician manages referrals and service requests, assesses individuals of all ages, and collaborates with physicians for appropriate care decisions. This role assists with discharge planning under the Director of Admissions and Referrals while serving as the primary community contact to provide essential resources and support. Additionally, the position supports the case management program within a 161-bed acute care facility.
