Lead Practitioner

The Lead Practitioner oversees the day-to-day functioning of the Street Based Lives Project and specialist service delivery to ensure a positive working environment and quality service. This role involves implementing the project, coordinating partnership meetings, and managing a caseload of service users with comprehensive assessments and support plans. The position requires collaboration with partners and agencies to improve outcomes for individuals facing challenges around mental health, substance use, housing, or domestic abuse.
