About this role

The Licensed Practical Nurse (LPN) will provide home health care to 5–6 patients per 8‑hour day, performing assessments, medication reconciliation, simple wound care, and patient/family education. The role works collaboratively with RNs, NPs, MSWs, a Chaplain, and office staff, utilizing a tablet, iPhone, and the HCHB electronic medical records system. New hires receive comprehensive onboarding, including Teams‑based orientation and several weeks of hands‑on precepting before transitioning to independent visits.

Responsibilities

Provide nursing care and assessments for 5–6 home health patients per day
Conduct medication reconciliation, wound care, and patient/family education
Collaborate with RNs, NPs, MSWs, chaplain, and office staff to coordinate care
