About this role
A Licensed Practical Nurse (LPN) at Mass General Brigham Home Care provides community-based skilled nursing services to homebound patients, supporting post-acute recovery and chronic disease management. The role works collaboratively with RN Case Managers to deliver care, document visits electronically, and support patients and caregivers across a defined local territory. The position is full-time (40 hours) with a primarily day schedule and a hybrid work arrangement.
