LPN- Med Room

The Licensed Practical Nurse provides basic assessment, planning, and execution of interventions for participants in the PACE clinic and their homes. This role involves administering medications, implementing primary care orders, and evaluating participant complaints both physically and by telephone. The nurse also educates participants and caregivers on care plans and communicates patient conditions to the interdisciplinary team.
