About this role
The LTSS Service Coordinator supports members enrolled in long-term services and supports programs by facilitating person-centered planning and coordinating integrated care across physical and behavioral health domains. This primarily field-based role serves members in Brevard, Osceola, and Seminole counties and often interfaces with clinical teams, providers, and community supports. Bilingual English/Spanish candidates are strongly preferred.
