About this role
The LTSS Service Coordinator (Case Manager) provides service coordination for a defined caseload in the IN PathWays for Aging program, working with members to document preferences, needs, and goals. The role facilitates Person Centered Planning, collaborates with interdisciplinary teams and medical directors, and manages integration of physical, behavioral health, and long-term services and supports. This is a field-based position supporting members in Daviess County, Indiana with occasional office attendance.
