The LTSS Service Coordinator is a field-based case manager role at Elevance Health serving members across Collier, Desoto, Glades, Hendry, Lee, and Sarasota counties in Florida. The position supports long-term services and supports (LTSS) programs and works with interdisciplinary teams to support member health and well-being.
