About this role
The LTSS Service Coordinator (Case Manager) is a field-based role supporting MyCare Ohio members by coordinating person-centered long-term services and supports across physical, behavioral, and psychosocial domains. The position is trauma-informed and culturally competent, operates primarily in the field across specified Ohio counties, and works with interdisciplinary teams and community partners to support member needs. A BA/BS is required.
