About this role
The LTSS Service Coordinator conducts service coordination for a defined caseload in specialized long-term support programs, meeting face-to-face with members across an assigned territory. They facilitate Person Centered Planning, develop comprehensive support plans and backup plans, and coordinate physical, behavioral health and LTSS needs in collaboration with members and interdisciplinary teams. The role is field-based and requires travel to client sites with occasional office attendance.
