About this role
The LTSS Service Coordinator – RN Clinician is a field-based registered nurse responsible for managing members' cases within the scope of licensure, developing and updating person-centered care plans, and optimizing member health across the care continuum. The role involves conducting telephonic and face-to-face functional assessments, coordinating services and authorizations, and working with interdisciplinary teams to address physical, behavioral, and social needs. The position serves members in Orange, Lawrence, or Crawford County and requires travel to client sites with occasional office attendance.
