Managed Care Nurse

This role serves as a patient advocate to facilitate and coordinate the transition of patients to the next level of care in accordance with applicable laws and agency policies. The professional collaborates with inpatient facilities, discharge planners, and internal case management staff to evaluate inpatients referred for transitional planning or home health services. Daily functions are performed through telephonic outreach and collaborative efforts to ensure seamless care transitions.
