Hartford HealthCare is seeking an Advanced Practice Provider to join the Clinical Integration team as a Manager, Value-Based Care, Risk Adjustment & Quality. This role provides clinical, operational, and strategic leadership to advance value-based care performance, risk adjustment accuracy, quality improvement, and documentation integrity across the Clinically Integrated Network. The position involves overseeing the Risk Adjustment Coding and Education team, driving provider engagement, ensuring regulatory compliance, and collaborating with leadership to improve population health outcomes and financial performance under value-based contracts.
