MDS Care Plan Coordinator

The MDS Care Plan Coordinator performs nursing assessments and documentation review to complete resident MDS, CAAs, and Care Plans in compliance with nursing standards and regulations. This role involves leading the Interdisciplinary Team through the assessment process, coordinating care plan completion, and ensuring accurate documentation and signatures. The position also requires monitoring regulatory reports, assisting with audits, and educating facility staff on RAI process compliance.
