MDS Coordinator

THE FORUM is seeking a hospitality-focused MDS Coordinator to oversee the supervision, coordination, and timely completion of MDS assessments, care plans, and medical records for residents. This full-time role requires an RN to ensure compliance with HCFA and Medicare guidelines while facilitating effective communication across shifts and departments. The position also involves participating in care plan conferences and supporting resident care as needed.
