Medical Assistant Pilot

The Medical Assistant Pilot supports the implementation and evaluation of a pilot Chronic Care Management (CCM) program by providing patient outreach, chronic disease education, care coordination support, and documentation services for eligible patients with chronic health conditions. This role helps improve patient adherence, identify barriers to care, support quality outcomes, and ensure documentation aligns with CCM program requirements. The position contributes to clinician-guided at-home rehab and patient-centered care by helping patients remain engaged in their long-term health management plans.
