Medical Assistant

This role involves providing telephone triage, patient follow-up, and medication refills while coordinating care and tracking patient recalls. The position requires assisting with outreach activities, coaching patients on chronic health condition management, and ensuring timely patient flow through effective documentation and collaboration. The incumbent will also act as a float for in-person clinical care during staff shortages and participate in quality improvement initiatives.
