Medical Case Manager

As a Care Coordinator/Medical Case Manager, you will work with clients of all ages living independently or in assisted communities who have complex health needs. You will create a Health Action Plan with the client, meet with them monthly in their home to focus on their goals, and engage with healthcare team members to coordinate care needs. This role involves linking clients to community resources, promoting skillset growth toward recovery, and supporting them to navigate and independently access healthcare resources and services.
